Terms & Conditions – Free Eligibility Assessment

Dr Rylan Hayes – The Vision Surgeon offers a complimentary Laser Vision Correction Eligibility Assessment (the “Assessment”) through Lasersight Sunshine Coast under the following terms and conditions:


1. Scope of the Free Assessment

  • The Screening assessment is conducted by our in-house optometrist, is free of charge and includes:
    • Ophthalmic screening scans and advanced eye measurements and mapping.
    • A preliminary evaluation and discussion regarding suitability for laser vision correction procedures (including LASIK, PRK, or other refractive surgery).
  • The purpose of this Assessment is to provide preliminary information only about potential suitability for refractive procedures.
  • It does not include any appointments with Dr Rylan Hayes directly nor any procedures, treatment, nor surgery.

2. No Obligation

  • Undergoing the Assessment does not create any obligation to proceed with surgery, book further appointments, or engage in any treatment with Dr Rylan Hayes

3. Further Consultations and Costs

  • If you choose to proceed beyond the Assessment (e.g., booking a consultation with Dr Hayes, undergoing further diagnostic testing, or scheduling surgery), additional fees may apply.
  • Any treatment, procedure, or specialist consultation beyond the free Assessment will be charged according to our standard fees, which will be discussed with you in advance.

4. Findings Requiring Treatment

  • If the Assessment or scans identify any other eye condition (whether urgent or non-urgent), further investigations, management, or treatment may be required and recommended.
  • Such management may incur additional costs not covered by the free Assessment.

5. Limitations

  • The Assessment is intended only as an initial suitability check.
  • A final decision regarding your candidacy for laser vision correction can only be made following a comprehensive consultation with the surgeon.

7. Right to Withdraw or Modify Offer

  • We reserve the right to withdraw, suspend, or modify this free Assessment offer at any time without notice.
  • Any bookings made before such withdrawal will be honoured under the conditions current at the time of booking.
